For two years the argument for compounded semaglutide was simple: the approved drug cost over a thousand dollars a month and the compounded version cost two hundred. That gap has closed from both ends.

Brand Wegovy fell to $349 through NovoCare. The approved oral tablet arrived at $149. Meanwhile compounded programmes commonly land at $178 to $199 all-in once a membership is added, and several are dose-tiered so the entry rate is not the maintenance rate.

An approved product with its own trial programme is now the cheaper option for many buyers. That is the single most useful fact on this page and it appears on almost no comparison site, because most were written when the opposite was true and have been updated only on price, not on conclusion.

Where the low compounded figures come from

Figures in the $99 to $149 range are real but usually conditional. In our experience checking them, they are one of four things:

• An annual prepayment. One provider's $99 is billed yearly at $1,188 up front.
• Medication only. A $99 rate with a mandatory $79 membership is $178.
• A starting dose. Dose-tiered programmes quote the lowest step.
• A first-month promotion. One provider's page shows $99 first month against $280 ongoing.

The salt-form question, unique to compounded semaglutide

Semaglutide sodium and semaglutide acetate are not the same active ingredient as the semaglutide base in the approved products, and the FDA has said they are not appropriate for compounding. Ask for the exact salt form and concentration in writing before you buy.

What the cheapest route costs you in evidence

Every efficacy figure in this field — STEP, SELECT, SURMOUNT — comes from a trial of an FDA-approved product. None was collected on a compounded preparation, a microdose or an orally disintegrating tablet.

That does not make compounded preparations ineffective. It means the evidence you are being shown was gathered on something else, and the strength of that evidence does not travel with the molecule across a change of formulation, dose or route.

At $149 for an approved pill against $178 for a compounded injection, the trade is no longer cost against evidence. It is a preference about route.
