The approval

The FDA approved Foundayo (orforglipron) on 1 April 2026 for adults with obesity, or overweight with weight-related medical problems, alongside a reduced-calorie diet and increased physical activity. Prescriptions were accepted immediately through LillyDirect with shipping from 6 April, followed by retail and telehealth availability.

It is the first small-molecule, non-peptide GLP-1 receptor agonist approved for chronic weight management. That chemistry matters practically: unlike oral semaglutide, which must be taken in the morning on an empty stomach with limited water and a 30-minute wait, orforglipron can be taken any time of day with or without food.

Orforglipron was discovered by Chugai Pharmaceutical and licensed by Lilly in 2018.

The efficacy figure, and the range around it

ATTAIN-1 randomised over 1,600 participants across eleven countries to various doses of orforglipron or placebo for 72 weeks, with percentage change in body weight as the primary objective.

The reported result varies by source. Lilly's own announcement cites an average of 12.4% at the highest dose. Secondary coverage frequently cites approximately 11%, and one telehealth partner quotes 11.2%. The variation almost certainly reflects different estimands — trials publish more than one analysis — but the sources do not consistently say which they are quoting.

We have recorded the range rather than a single number, and marked the record provider-reported pending capture of the ATTAIN-1 publication. Charting one figure when the analysis behind it is unstated would repeat an error we have already corrected once on this site.

Where it sits against the alternatives

On mean reduction, orforglipron is the lowest of the approved options: roughly 11 to 12.4%, against 13.6 to 16.6% for oral semaglutide in OASIS 4, about 13.7% for injectable semaglutide in SURMOUNT-5, and about 20.9% for tirzepatide 15 mg in SURMOUNT-1.

That is not a criticism. An approved product with premarket review, published trial data, a known safety profile and manufacturer supply is a materially different proposition from a compounded preparation with none of those things — and needle avoidance is a genuine barrier for a substantial group.

It is a correction to the framing that dominated coverage, which led on price and omitted efficacy entirely.

Pricing

Through LillyDirect: $149 at the 0.8 mg starting dose, $199 at 2.5 mg, $299 at 5.5 and 9 mg, and $349 at 14.5 and 17.2 mg — with the top two doses returning to $299 when refill timing requirements are met. Eligible commercially insured patients may pay as little as $25 with a savings card. Eligible Medicare Part D patients may access it for $50 from 1 July 2026 through the GLP-1 Bridge.

The $149 entry price undercuts most of the compounded market, where standard programmes run $169 to $399. Several compounded programmes cost more than this approved product at its starting dose.

Safety

Orforglipron carries a boxed warning for thyroid C-cell tumours including medullary thyroid carcinoma, and should not be used by patients with a personal or family history of MTC or with Multiple Endocrine Neoplasia syndrome type 2. Adverse effects are predominantly gastrointestinal — nausea, vomiting, diarrhoea, constipation, abdominal pain — and it is designed for gradual dose escalation to limit them. Concomitant use with other GLP-1 receptor agonists is discouraged.

What it changes about the market

Three things. It removes needle avoidance as a reason to stay off approved products. It establishes a $149 price anchor at the approved end of the market. And, as a small molecule, it is easier to manufacture than an oral peptide — which matters for supply resilience in a category defined for three years by shortage.

For anyone currently paying more than $149 for a compounded preparation, this approval is the strongest argument yet for re-running the comparison. For anyone at a maintenance dose where flat-rate compounded pricing genuinely wins on annual cost, the trade is now explicitly efficacy against price rather than price alone.
